Manage Patients

Chronic Kidney Disease (CKD) is best identified and treated early as part of comprehensive primary medical care. Goals of medical management are to: identify and treat, monitor, slow progression, and reduce and treat complications including functional status and quality of life issues. Additional goals include providing culturally appropriate patient and family education about CKD, diet, lifestyle, and self-management support.

Some people will progress to kidney failure. Early education about treatment options may be beneficial; patients and their families may be better prepared to face kidney failure. Continuity of care or assessment may involve collaborating with a nephrologist or pediatric nephrologist. As with any chronic condition, a multidisciplinary approach is preferable. Collaboration among primary care providers, dietitians, nephrologists, nurses, cardiologists, pharmacists, diabetes educators, medical social workers, and others may optimize patient care.
